[Bug Report] Players are trapped in a dungeon after death + Dungeons are no longer recognized
When players die in a dungeon set to no respawns/spectating, they respawn back in the dungeon and are trapped. It seems that the world never gets unloaded, and MythicDungeons forgets that it is a dungeon world. They cannot use any commands, and /md leave states that you are not in a dungeon. It also seems like this only applies on deaths - if a player successfully completes the dungeon, expected behavior is seen.
Expected Behavior: The player should respawn back into the location and world they first ran /md play at. The dungeon instance should be cleaned and removed.

ADDITION: Dying at least once in a dungeon with multiple lives causes anything - loot chests, triggers, /md leave - to not work at all. If there's a party in the dungeon, only the players who have died once are affected - the dungeon works fine for anyone else who has not died yet. I assume this is also related to this issue, due to MythicDungeons failing to recognize the dungeon after a death?
