This issue relates to Citizens NPCs not spawning in dungeons when they are far from the dungeon spawn point. This issue even occurs with proximity based spawning. For example, if a trigger is set to spawn an NPC when the player is within 5 blocks. Spawning NPCs via any trigger only works when the NPC is close to the dungeon spawn point.

As per our discussion, this may be related to the 1.21 update.

This is somewhat still relevant, I couldn't reproduce it 10/10 times but it did happen regularly

Just wanted to add that many developers have expressed issues with citizens. I highly recommend using mythic for npcs. Mythic can do everything that citizens can do, due to this I will be marking this as low priority but we will probably take a look at it.
